The family of a man who died on July 4th at the Phillips 66 refinery in Belle Chasse is seeking answers as to how the accident happened and why his body took days to be recovered. According to a Phillips 66 spokesperson communicating with NOLA.com, the accident occurred in a cooling tower that had to be shut down before first responders could enter. OSHA is also looking into the death.
