The Insurance Council of Texas says that even after five years of investigations, no one has yet been charged for a Texas fertilizer plant explosion that killed 15 people in West, Texas. The Texas State Fire Marshal’s Office (SFMO) and the Alcohol, Tobacco and Firearms (ATF) determined the fire that caused the explosion was intentionally set in the seed storage room of the West Fertilizer Plant. Further, the plant carried only $1 million in liability coverage while damages totalled over $100 million. OSHA also discovered numerous severe safety violations at the location.
