A defective crane at a petrochemical facility in Vidor, Texas resulted in the death of one worker and $109,000 in penalties. OSHA issued a willful violation to Trans Global Solutions for operating a defective crane, plus seven serious violations for failing to conduct inspections and address electrical and drowning hazards. According to the release, while moving hoses and trying to lower the crane’s boom, the crane’s remote control failed and the operator was unable to control the boom’s movement, which led to a worker being pinned and killed.
