An Arkansas steel wire company received $124,710 in OSHA fines after an employee suffered amputation while trying to untangle moving wire. Investigators discovered that the Bekaert Corp. facility in Van Buren had machines and pulleys carrying steel wire with unguarded pinch points, a violation for which OSHA cited the company at the same location in April 2015.
