OSHA cited San Antonio-based Longhorn Contractors yesterday for three serious, two repeated and one other violation, following the death of a worker over the summer who fell 35 feet from a roof without fall protection. OSHA issued the serious violations for failing to perform inspections, failure to provide proper fall protection equipment and failure to provide prompt medical attention. The agency cited the repeated violations for not enforcing the use of fall protection or providing fall protection training. The other violation was for failing to report the fatality within 8 hours. Over two separate worksite inspections, the company has been ordered to pay $80,190.
